New Vision Hospice Referrals

Hospice care referrals in Southern California

Right fit,
right now.

When someone you love needs hospice, you should not have to call ten agencies from a hospital hallway. Tell us about them once. We match your family with a licensed hospice that fits their care needs, their insurance and their neighborhood — in most cases the same day.

  • No cost to families
  • Medicare-certified agencies only
  • A person answers, 7 days a week

Every match checks three things

  • Care needsDiagnosis, symptoms, equipment, language
  • CoverageMedicare, Medi-Cal or the plan they have
  • LocationHome, hospital, assisted living or board & care

Licensed agencies in good standing, nothing else

Who we help

One call, whichever side
of the bedside you are on.

Families, the clinicians discharging a patient, and the facilities caring for residents all need the same thing: the right hospice, quickly, without a week of phone tag.

Families

You just heard the word “hospice.”

We walk you through what the Medicare hospice benefit covers, what to ask an agency, and what happens on the first visit. Then we connect you with one that can start.

  • Plain answers, no pressure
  • Agencies that visit your parent’s area
  • Someone to call back when new questions come up

Discharge planners & physicians

An accepting agency before the bed is needed.

Send the face sheet and the order. We confirm an agency that has accepted the patient, can take their payer, and can do the admission visit on your timeline.

  • Same-day discharges
  • Accepting agency confirmed back to you
  • One contact for every referral

Facilities

Residents who can stay where they live.

Skilled nursing, assisted living and residential board & care homes: we match residents with hospices that already visit your building and work well with your staff.

  • Agencies familiar with your facility
  • Coordination with your care team
  • Family kept informed throughout

How it works

Three steps. Usually one day.

  1. Minute one

    You call us

A care coordinator picks up — not a call center. Tell us who needs care, where they are today, and what the doctor has said. Five minutes is usually enough.

  2. Within the hour

We match by care needs & insurance

We check which licensed hospices serve their address, accept their coverage and can meet their needs — oxygen, wound care, dementia, a language at the bedside — and we confirm who can admit.

  3. Same day, in most cases

    Placed within hours

The agency calls you to set the admission visit. A hospice nurse comes to the home or facility, and we check back with you afterwards to make sure it is the right fit.

Start with a call

Once a physician has ordered hospice, admission can often begin within hours.

Our provider network

Local agencies we know,
not a list off the internet.

We refer only to hospices that are licensed by the California Department of Public Health and certified by Medicare, and we keep track of which ones are answering, admitting and visiting where.

Hospital and skilled-nursing teams: one call gets a coordinator working on your patient while you finish the paperwork. We confirm the accepting agency back to you by phone.

Helpful to have ready

  • Patient name and date of birth
  • Diagnosis and the hospice order, if written
  • Insurance: Medicare, Medi-Cal or plan name
  • Where they are now, and where they will go
  • A family contact and the best time to call

Missing something? Call anyway. We can start with a name and a phone number.

What families ask

The questions everyone
is afraid to ask.

Short, honest answers. If yours is not here, call — asking costs nothing and commits you to nothing.

Ask us directly
Who qualifies for the Medicare hospice benefit?

Someone with Medicare Part A whose doctor and the hospice medical director certify that, if the illness runs its normal course, life expectancy is six months or less — and who chooses comfort-focused care instead of treatment to cure that illness. It is not a countdown: people who live longer can be recertified and keep receiving care.

What does hospice cost our family?

For people on Medicare, hospice care itself costs nothing. There can be a copay of up to $5 for each prescription for pain and symptom relief, and a small coinsurance on the Medicare-approved amount for short inpatient respite stays. Medicare does not pay room and board in a nursing home or assisted living. Medi-Cal and most private plans also cover hospice. Our referral service is free to families.

How fast can hospice start?

Once a physician has ordered hospice, many agencies can do the admission visit the same day or the next morning. Calling early matters: the extra days give time for equipment, medications and support to be in place before a crisis, not after one.

Can hospice come to a home, assisted living or nursing home?

Yes. Most hospice care happens wherever the person already lives — a private home, a family member’s home, assisted living, a residential board & care, or a skilled nursing facility. The team comes to them: nurse, aide, social worker, chaplain and volunteers.

Can we keep our doctor, or change our mind later?

Your loved one can keep their own doctor as the attending physician. They can also stop hospice at any time and return to regular Medicare coverage, or switch to a different hospice agency once per benefit period. Choosing hospice is a decision about comfort, not a door that locks.

Why use a referral service instead of calling an agency?

You can call an agency directly. What we add is the comparison: which agencies actually serve your address, take your coverage, have capacity this week and can meet specific needs. That is a dozen phone calls you do not have to make while you are trying to be at the bedside.
